The members of Still Bay Branch had the privilege of learning about a very important document that will be of assistance to one’s loved ones should you pass away or become ill. The document is called the LIFE FILE/LEWENSLÊER.
This document has various sections in which information is grouped. The document can have both you and your spouse’s information. One can decide how you would like to record the information, together or separately. You may decide which documents will be originals or copies. This file should be saved in a safe place and your wife or next of kin should know where it is kept.
- PERSONAL INFORMATION
- ID document or card
- Passport – if you have one
- Drivers’ license
- Marriage Certificate
- Marriage Contract
- Birth Certificates or Death Certificate should you have lost a spouse
- Your next of kin information. Husband, wife, children etc.
- The church you belong to and the contact details
- ESTATE
- Last Will and Testament
- Your Executors ID document or card
- Executor’s details
- Living Will
- ASSETS AND LIABILITIES
- List of fixed assets
- List of immovable assets
- Bond details
- Title Deeds
- Municipal Account
- Telephone/Cellular phone Account

- Information on Motor Vehicles – Make and model of the car, license documentation, roadworthy, Hire Purchase Agreements

- Your brokers details – name and contact details
- Bookkeeper details – name and contact details

- Short Term Insurance – householders’ insurance
- Long Term Insurance – Life Policies, Living Annuities etc.

- Any cash that has been held in case of emergencies.
- MEDICAL INFORMATON
- Details of your General Practitioner – name and contact telephone number
- Medical History
- Medication that you are using
- Medical Aid details
- Emergency Contact details
- PASSWORDS AND PIN NUMBERS
Although this information should be kept secret and memorized, there might be instances when it will be required by people other than yourself during emergencies. List the passwords in this section. One could list them, seal in an envelope and file under this section.
- OTHER INFORMATION
- Funeral Home that must be used
- Tax Information – Tax number and latest assessments
- Any information pertaining to the business you are involved with. Example – partnership agreements, company documents etc.
- Rental Agreements
- Any other accounts that you might have.
OTHER SUGGESTIONS
- Place a record of your doctor’s details, medical aid, medication you are on, medication that you are allergic to and contact details of a family member in the cubby hole of your car. This could be required in the event of an accident.
- Prepare a small emergency suitcase or bag with clothes, pajamas, slippers, toiletries etc. that you would need should you be admitted to hospital in a hurry
